I also use the phrase "root cause analysis" it's when you dig deep into an issue to figure out what led to an issue. Usually done for ongoing/chronic or critical issues.

I also may have also used People, Process, Technology.
When doing a deep dive we look for multiple points of failures.
Example new person makes a mistake and it wasn't caught1. Training might be an issue.
Technology should have caught or stopped
Process may not be documented or have check points

You also use "the ask" as a noun. When I was growing up ask was a verb.

I think "the ask" means a request.

What are you asking for. Usually, you want something specific.

It is used when someone comes knocking, telling a story, ..at the end, it is important to outline what you are seeking, even if it is just more time
